The INCAREHEART (Pre-commercial Procurement of innovative ICT-enabled integrated care solutions to advance multidisciplinary health and care for patients with chronic heart failure) project, coordinated by the Swedish Jämtland Härjedalen Regional Management with partners from Turkey, Greece, Portugal, the Netherlands, Spain and Germany included in the consortium, is a European Union project that has been carried out since February 2021, funded under the Horizon 2020 Programme 'SC1-BHC-20A-2020 - Pre-commercial procurement (PCP) for integrated care solutions' call heading. The project is a PCP (Pre-Commercial Procurement) project and has a tender budget of 4.6 million Euros.

It aims to ensure that chronic heart failure patients can be monitored remotely by healthcare professionals using an online platform and that these patients are more involved in their own disease management processes and thus more empowered (patient empowerment), ultimately resulting in better quality and more continuous management of their chronic diseases. To facilitate this, software programmes with artificial intelligence will be developed, that will provide better management approaches for chronic diseases and will be fully integrated with the health information systems of the pilot regions. Research and development of innovative solutions will be transferred to contractors in accordance with the European Union's PCP procurement model.
