Scientific Outputs

The main objective of POWER2DM is to develop and validate a personalized self-management support system (SMSS) for Type-1 and Type-2 diabetes patients that combines and integrates:

  • a decision support system (DSS) based on interlinked predictive computer models; KADIS, T2D-Marvel, and state-of-the-art risk scores.
  • automated e-coaching and advice functionalities based on Behavioural Change Theories; and

real-time personal data processing and interpretation.

Tackling ‘grand’ or societal challenges

The approach of POWER2DM is motivated by the following needs to improve existing diabetes care and prevention services and approaches:

  • Need to overcome limitations of existing DSS solutions that do not sufficiently target patients and have a limited focus mainly concentrating on Blood Glucose Control
  • Need to reduce the gap between evidence-based findings and diabetes care in general practice
  • Need for self-management interventions that are able to maintain efficacy over longer periods of time
  • Need to reduce time delays involved in finding optimal therapy

Need for research on complex evidence-based models used for patient-targeted decision support

Industrial Innovation (including innovation in services as well as products and processes)

The extent to which POWER2DM will reach these objectives and empower diabetes patients to become co-managers of their own health will be evaluated in 3 pilot studies in 3 different European countries; Germany (IDK), Netherlands (LUMC), Spain (SAS).

Pilots will evaluate the system both in terms of the clinical and cost effectiveness of new diabetes care approach and technologies used. In this respect, we are expecting improvements in diabetes care process and prevention of diabetes and its complications.

Research-influenced changes in policy, agenda-setting

The project influence the research in Turkey regarding chronic disease management and behavioral change issues. We are planning to engage with the new PCP project,  PROEMPOWER (http://www.proempower-pcp.eu/), coordinated by Ministry of Health Turkey, which aims to realize a disease management platform for Diabetes.

The provision of Improved Public Goods

The public health organizations in Turkey may benefit from the new diabetes care approach and technologies of the POWER2DM project to improve the care processes including the Family Practitioners and other healthcare organizations.

The improved exercise of professional skill

New diabetes care approaches may improve the exercise of special professional skills like Diabetes Nurse or Health Behavior Change coaches.

Human capital development

The increasing number of applications and care programs in chronic disease management, patient empowerment and behavioral health change may improve the human capital development (application developers, physicians, nurses, dieticians, psychologists, etc).
